Referrals and requests for St. Mary's Home Health and Hospice Services, Inc. come from physicians, nurses, social workers, discharge planners, community agencies, family members, as well as the patient themselves.

Once the referral is received, a member of the St. Mary's staff will obtain the necessary information and speak with the physician, patient and the family to determine what services are appropriate and necessary.
